The New Grief: How Modern Medicine Has Transformed Death and GriefThe realities of death and dying have changed profoundly in a relatively small period of time. Why? Thank the ongoing and remarkable advances in medical diagnosis and treatment. As a result of these advances, life expectancy in countries like ours continues to grow. We all die, but modern medicine is getting better and better at staving off death. And because of this the nature of grief has changed.

In her groundbreaking 1970 book, On Death and Dying, Elisabeth K?bler-Ross identified a process which she believed individuals pass through when they are confronted with death. At the time, sudden and unexpected death was much more common than it is today. The grief associated with that kind of loss is captured powerfully in Joan Didion?s memoir, The Year of Magical Thinking, which recounts her reactions to sudden death of her husband, who collapsed and died of a heart attack in the midst of eating dinner. Didion?s initial response to her husband?s death typifies what K?bler-Ross called denial. She refused, for example, to read his obituaries. She refused to throw away his clothes. And she avoided going places that would remind her of him.

Contrast the above to the tale told by Eleanor Clift in her book, Two Weeks of Life: A Memoir of Like, Death, and Politics. There she details her experiences after her husband, Tom, was diagnosed with kidney cancer, fully five years before he died. Tom spent the last ten weeks of his life at home, in a bed that hospice services had set up for him. The couple had a excellent thought, at least four months earlier that the end was finally approaching, when Tom?s oncologist recommended discontinuing chemotherapy. Even then another four months transpired.

Clift?s memoir ? like Patti Davis?s account in The Long Goodbye, which recounts the decline and death of her father, President Ronald Reagan ? describes what more and more people are facing: the profound shift in the experience of dealing with dying and death.

A Black Hole

This shift in the nature of dying in turn has made a profoundly different experience from what we have come to call mourning, which starts after the death of a loved one. This new grief starts when we learn that a loved one has been diagnosed with an illness that is terminal or life-threatening. Some have likened this to ?falling into a hole with no thought of which way to turn.? Others have described the new grief as a process of ?catapulting from crisis to crisis with no one to turn to other that whoever happens to be your doctor at that moment.? This marks the beginning of a journey that may last months or years, and which has the potential to affect just about every aspect of our lives and our relationships.

As useful as the K?bler-Ross model may have been in its time, the new realities of death and dying demand a new model?one that can help patients and loved ones alike know and navigate their way through a very different set of circumstances. Today, having a loved one live with a terminal diagnosis for an extended period of time is increasingly replacing sudden and unexpected death as the norm. Consider, for example, that two-thirds of those who are diagnosed with cancer currently have a five-year survival rate. Today, a diagnosis of cancer (or coronary artery disease) no longer means that death is imminent. In fact, over 1.4 million cancer survivors are more than 20 years past their initial treatment episode. Of these, approximately 20 percent will experience a recurrence of the cancer (usually in a different organ) and the whole grueling process will repeat itself.

One profound result of all of this is that death has become less and less a sudden and unexpected event. In its place has come a process that starts with a diagnosis, proceeds through a period of treatment (or treatments), and ends eventually in death. This process means that both the terminally ill individual and the family are increasingly confronted with the need to ?live with death? for a prolonged period of time.

Regardless of whether it comes suddenly and unexpectedly, or slowly with much advanced notice, the death of a loved one leaves us with a feeling of loss. That is unavoidable, given our human capacity to form attachments. In a sense, our attachments define us. When we lose an attachment, we lose a part of ourselves. And we grieve that loss. That said there is also a case to be made that the new grief differs in significant ways from traditional grief, not the least of which is that it includes the terminally ill person. What has increasingly become a protracted process, as opposed to an event, typically draws in the entire family of the dying person for months or even years.

A Road Map for Death and Dying

Drawing on personal experience as well as our professional work, Dr. Barbara Okun and I set out to see if could not make that ?road map? that families can turn to as they navigate their way through the current realities of death and dying. We and our associates also conducted many in-depth interviews with family members who had first-hand experience with this emotional rollercoaster. That road map is presented in Saying Goodbye: How Families Can Find Renewal through Loss.

The challenges that families must face when confronted with a terminal diagnosis of a loved one are complex. They include evolving new structures and dynamics as the person they like slowly slips away. It means navigating your way through a fragmented medical system which often lacks communication, much less coordination. It means learning how to cope with setbacks and deterioration, as well as periods of seeming remission. It means dealing with the complexities of extended grief, which can wear individuals down and lead at times to ambivalence about the wisdom of extending life. It means talking with a dying loved one about mortality and other issues that do not arise when death strikes suddenly and unexpectedly, to insure that when death does come it is with grace and dignity. It means learning to make space for extended grief in lifestyles that are busier than those generations that went before us.

The new grief also involves confronting family issues that may have been dormant?but unresolved?for many years. These issues typically reemerge as families go past their initial reactions to a terminal diagnosis and are forced to interact and work together more through a process of extended grief. Finally, the new grief means moving forward together, potentially as a stronger and more resilient family, after our loved one passes.

The new grief is neither pretty nor orderly. What we share in our book, but, is some of the wisdom we have gleaned from those who have walked this path that we all will walk ? if we have not already. It is, in effect, the book that Dr. Okun and I wanted to have when we were there.

Bulger prosecutors want more time for discovery (Reuters)

BOSTON (Reuters) ? Government attorneys working on the James "Whitey" Bulger case asked on Friday for more time to go through mountains of evidence against the former Boston mob boss before turning the materials over to the defense.

In a court filing, federal prosecutors said the usual 28-day period to produce discovery was insufficient since many of the documents, photographs, videotapes and other evidence had been boxed and stored for years, and in some cases organized for use in other related cases.

Producing the materials for Bulger and his defense team requires the reproduction of "thousands of hours of audiotapes and videotapes as well as hundreds of photographs," according to the filing.

"Obviously, the defendant, who has been a fugitive for over 16 years, bears some responsibility for this predicament," the government said.

Prosecutors asked for an extension until August 31.

Bulger, the former leader of the Winter Hill Gang, pleaded not guilty earlier this month to all charges against him, including 19 alleged murders from the 1970s and 1980s.

The case against the 81-year-old gangster could be a years-long judicial process. The first status hearing in the case is scheduled for September 14.

Bulger and longtime girlfriend Catherine Greig, 60, were arrested in Santa Monica, California on June 22.

Bulger fled Boston in late 1994 after receiving a tip from a corrupt FBI agent that federal charges were pending. Greig joined him a short time later and has been charged with harboring Bulger as a fugitive.

The Food Canon: Tom Yum Goong (Thai Spicy Prawn Soup)

I have been hoping to write up a recipe on Tom Yum Soup, a thought which I had already mentioned in the first article of this blog.

Last Sunday, as usual, I was reading and enjoying Christopher Tan's "Ask a Foodie" column. I nodded in agreement when I read his article on making a good Tom Yum. You can find out more about Chris' work here. And this morning, I read in the papers that it is no 7 on CNN list of the world's 50 most delicious food.

I have always been fascinated by a good bowl of Tom Yum Soup. While on my honeymoon in Phuket 18 years ago, I enjoyed a bowl of Tom Yum Goong with steamed rice. That experience has never left me. The problem is that most of the variations of Tom Yum which I had since has not come close to that. These variations always seem to have so much in them that they crowd out the real Tom Yum, leaving only faint echoes of the one I had.

I reckoned that I simply needed to demystify this dish myself if I want to have it on my table. When I was in Banchang last year (an hour's drive from Bangkok), I visited the local wet market. It was very clean and the herbs and spices were displayed neatly. I saw many packets of the "Tom Yum trio" and I bought a bagful. That began a series of experiments before I got it right. It would have helped if I had a copy of The Principles of Thai Cookery by Chef McDang then or read this article by Chris Tan.

I made it again today. I have been having a cold for more than a week now. When your nose is stuffed up, nothing like a good Tom Yum Soup to clear the sinuses. The recipe is actually quite simple.

This soup consists of these main ingredients:

1. Stock: this can be any kind of meat or seafood.
2. Trio of herbs - lemon grass, galangal (blue ginger) and kaffir lime leaves,?
3. Spices: chillies, fish sauce, lime juice (or tamarind)
4. Condiments: prawns, meat, straw mushrooms or bamboo shoots - stuff you chew on when slurping the soup. Unless you are a cow, you leave the herbs alone. ? ?

This soup is basically an infusion. The heart of it is the trio of herbs (see main photo). If you can, get hold of fresh ones. Get the Thai version of galangal which is smaller and thinner and has a more fragrant and less bitter taste cf to the Malaysian version which is larger (and also looks hideous). As for Kaffir lime leaves, I have found the Thai ones to be more fragrant and the colour slightly lighter green. You can get this at Golden Mile Complex.

As for the stock, I prefer using Tom Yum Gung (Prawns). Just boil the shells. I prefer to blend the heads and shells using a stick blender. This part is very similar to how I prepare my Prawn Mee. Now, if you can lay your hands on Freshwater Prawns (see some pics here), this flavourful prawn, especially if caught wild, can just be added whole to the soup before you serve. But since we do not have them here, squeezing the life out of the normal prawns is the way to go.

I would normally use bamboo shoots, which goes well with the soup. I will try out the fresh straw mushrooms as suggested by Chris. As for fish sauce, I like the Vietnamese variety, which is fragrant and less salty.

Ok, let's go. As usual, use this recipe as a guide.


Tom Yum Goong (Thai Spicy Prawn Soup) Recipe

Ingredients

Serves 6

Half a kilo medium size prawns, peeled and deveined.
8 bowls of water
6 slices of fresh galangal (can use more, depends on size)
2 stalk of lemon grass, bruised (purple part only)
6 kaffir lime leaves.
10 chilli padi (small chillies), bruised
Limes (the larger Thai variety)
Straw mushrooms or Boiled bamboo shoots
Fish Sauce to taste
Cilantro or Mint leaves for garnishing

Method

  1. Bring water to boil in a pot
  2. Put the prawn heads and shell in. Blend (optional) and boil for 10 minutes.
  3. Strain the soup
  4. Add the trio of herbs to impart the scent. This will be quick 30 seconds or so.?
  5. Add the chillies. Season with fish sauce (to impart salinity or saltiness).
  6. Add the mushrooms and boiled bamboo shoots. 1 minute.
  7. Add the prawn meat and switch off the fire. It is important not to overcook the prawns
  8. Squeeze lime juice (use the large Thai ones, not calamansi) into the serving bowls (quarter to half a lime each)
  9. Scoop soup into the bowls, garnish with cilantro or mint leaves if you like and serve immediately.
As you can see, you just need to be organised and prepare the steps beforehand. I like the fresh lime taste that hits your face as you drink the soup and so, I prefer to add them at the last step, into the bowls, not the pot.If you infuse the soup for too long, the taste will get bitter or too intense. And don't overcook the prawns.You can cook and serve this just before the meal, If your stock is already prepared, you need only about 5 minutes to prepare and serve.

There was once when I used bottled calamansi, and that was disastrous as it was too intense and lacked the fragrance of the fresh Thai limes. On another occasion, I added squid. I was clumsy and as the heads were not cut out or washed properly, the soup quickly turned black. I learnt then that the soup needs to look as appetizing as it tastes and most of it was wasted as none in my family wanted Black Inked Tom Yum. Neither did I.

Tom Yum Goong is best enjoyed accompanied by steamed rice. It can also be a starter soup. It is best savored in individual bowls rather than served along with the rest of the dishes as is normally done on family tables. This is just a matter of preference.

Get the basics right - appreciate the fresh taste of the trio of herbs -? and you can experiment with other meats/veg but never crowd out the taste of the trio. Try adding squid, scallops or fish. You can opt to make a more intense stock. I also like to add a bit of chicken stock or bullion when finishing the soup. I sometimes use more prawns (especially when you blend the shells and heads) for a more prawny stock. The prawn based stock goes very well with the herby trio and if prawns are cheap and aplenty where you are, no reason why you should not be more generous with the use of prawns. You can use the heads and shells of 1 kilo of small to medium size prawns, and keep the meat for use in other dishes. Then use large prawn meat to serve. I sometimes do this if I am serving guests.

And don't even try to pair it with wine.?

America doesn't need a debt limit

The debt limit debate allows politicians to grandstand on fiscal responsibility. But the debt limit doesn't control how much America borrows. Congress does.

America's leaders need to get to yes on a budget deal ? one that marries substantial deficit cuts with a much-needed increase in the debt limit.

But that's not enough. Rather than merely increasing the debt limit, we should eliminate it.

I realize that sounds strange. With all the Sturm und Drang in the budget talks, you might think that the debt limit is essential to controlling Washington's profligate ways. It's not.

Washington has other tools for managing its finances. The annual budget process includes a series of steps by which Congress decides how much to spend and to collect in taxes. Those decisions determine the size of America's deficits and debt.

That simple fact often gets lost in the debate, so let me say it again: When Congress decides how much to spend and how much to tax, it is also deciding how much to borrow.

Unfortunately, the debt limit allows lawmakers to pretend that they can separate the two. Members routinely try to wrap themselves in the flag of fiscal responsibility by voting against debt limit increases. In most cases, though, those members have also voted for spending and tax policies that make those debt increases necessary.

Votes on the debt limit thus usually reflect raw politics, not substantive policy differences. Everyone knows that the debt limit has to rise. But they also know that voters hate debt. So law-makers jockey to see who can win the right to vote no and who must bear the burden of voting yes.

Democrats opposed debt limit increases when President George W. Bush was in office and Republicans controlled Congress. Republicans returned the favor under President Obama and the Democratic Congress. The only times we've seen hints of bipartisanship are when, as now, divided government has placed some responsibility on both parties.

A larger problem is that the debt limit institutionalizes risky brinkmanship. In divided government, both parties must agree to raise the debt limit. If they don't, the United States can't pay all its bills. We might even default on our debt. That's the economic equivalent of driving over a cliff.

Both sides would regret that outcome. But they face very different incentives. The party that holds the White House has to make sure that the government functions. That's why Treasury Secretary Timothy Geithner has repeatedly warned Congress about the damage that would result if the debt limit isn't raised in time.

But the opposing party wants to extract the highest possible price for agreeing to more debt. So they have to act as though hitting the limit is no big deal. That's why many Republicans have been discussing the potential to prioritize payments (putting interest first), and some have flirted with endorsing temporary default.

The problem with that strategy is that negotiations can fail, prioritization might not work, or we might be surprised with a sudden need for funds. In short, we might accidentally go over the brink.

Even if we don't, dancing on the edge is costly. Alone among major nations, the US talks openly about the possibility of default. Financial markets usually discount that rhetoric as mere politics. As the deadline nears, however, that rhetoric will sow doubt in financial markets, inspire warning shots from credit-rating agencies, and potentially increase our borrowing costs.

There's no reason to subject ourselves to those needless costs. The debt limit is an anachronism. Congress should eliminate it.

Evel Knievel: showman, rogue, reality TV star (omg!)

TOKYO (Reuters) - Daredevil Evel Knievel, known for his spectacular motorcycle stunts and equally outrageous crashes, was a flamboyant showman, a master salesman, and perhaps even the forerunner of reality TV.

What he wasn't, says biographer Leigh Montville, was all that likable.

"He was an outrageous character, an interesting guy to figure out. He'd been a thief, he'd been an insurance salesman, he'd been a bad guy," Montville said in a recent phone interview about "Evel: The High-Flying Life of Evel Knievel: American Showman, Daredevil and Legend."

"He was a rogue, and not so much a lovable rogue. Just a rogue."

But in his heyday, Robert Craig "Evel" Knievel -- who died in 2007 at the age of 69 -- strode the stage of celebrity like a superhero in a red, whited and blue leather jumpsuit with a cape, his hair sculpted back in a tall pompadour.

Among his more incredible feats was a 1975 attempted jump over 13 double-decker buses in London's Wembley Stadium and a failed effort to ride a rocket-powered motorcycle across the Snake River Canyon in Idaho in 1974. He walked away with minor injuries.

During his career he broke more than 40 bones in his body, including his back seven times.

Surprisingly, and perhaps a sign of different times, there was little fuss made about the basics of Knievel's act, which was basically a test virtually every time of whether he would live or die, Montville said -- and, as such, perhaps one of the first examples of reality TV.

"There was some talk about kids going out and breaking their collarbones and stuff, riding their little banana-seat bicycles and trying to push them over different obstacles," he said.

"But the basic concept of here's a guy, maybe he's going to die, let's tune in and see it -- and bring all the kids."

Born in Butte, Montana, Knievel said he was inspired at the age of eight when he saw an auto daredevil show and went on to live a personal life almost as painful as his job, including trouble with the law starting as a teenager.

He was dubbed "Evil Knievel" by a jailer in Montana after crashing his motorcycle while fleeing from police. He later changed the spelling to "Evel" as his daredevil career took off to avoid being perceived as a bad guy.

Among the tales that Montville tells is the fact that for all the stunts he pulled, Knievel's main money came from action figures -- a doll by any other name that was one of the bestselling toys for U.S. boys from 1973 to 1975.

"To kids he became an intimate figure like GI Joe or Barbie. They'd put him on the motorcycles and you revved it up and shot it off -- you could shoot it through fire, through puddles, through your sister's doll house," he said.

"Evel would fall off, but he'd be okay, and then you'd put him in your toybox at night. The toy was key to his whole operation financially."

This fell apart in 1977, when Knievel attacked a former promoter with a baseball bat and served several months in jail, losing most of his sponsorship. He lived in obscurity for close to the next 20 years.

Ultimately, Knievel's appeal was a combination of the ghoulish fascination of his stunts and a reluctant admiration for his daring, Montville said.

"He's the guy who jumped off the bridge, when the rest of us wouldn't jump. He's the guy who'd take all the double dares, and live fast," he added.

"Even the most sedate of us, in the back of our minds, say that maybe we should be doing that."

Acid reflux and GERD are essentially the same condition. Therefore, when we are talking about GERD medications we are also making reference to acid reflux medications.

There are several reasons to look at alternative methods for controlling severe heartburn symptoms. One is that some people want to avoid pharmaceuticals altogether. A second is that?long term use of GERD medications is addictive and even potentially harmful.

Unfortunately, when you enter into the world of alternative remedies you open up a new set of difficulties. There are numerous recommended solutions, schools of thought, and limited legitimate research. In many cases, the only way that you can know if something is going to work is to use yourself as the guinea pig.

In regard to acid reflux and heartburn, a simple internet search for natural remedies will uncover hundreds of possibilities. Some of recommended solutions appear illogical. For example, apple cider vinegar is one of the highest rated natural remedies. Besides the fact that vinegar tastes bad (by itself), it is also highly acidic. Pouring acid on acid doesn?t seem to make much sense. But there are many advocates who sing the praises of apple cider vinegar. Personally, I tried it, and it didn?t work for me. Based on the experience of others who swear by it, there is a possibility that it will work for you. If you can get by the taste?give it a try.

Closely related to apple cider vinegar is?pickle juice. Now, since pickles are made in a variety of ways the question arises, ?What kind of pickle juice?? Pickle juice advocates strongly favor juice from fermented pickles. I didn?t know there was such a thing?but I?ll take their word for it. Yet other suggest dill pickle or sweet gherkin juice. And if you can?t stand drinking the stuff straight?just suck on a pickle?or eat one. So they claim.

Likely a more long term natural fix will require a change in diet. The idea is to keep your system in a proper alkaline to acid pH balance. What we eat controls that balance and most of us, due to our highly acid modern diets, are leaning too heavily on the acid side of the scale.

For these diet changes to work you must decrease the consumption of acidic triggers like chocolate, caffeine, sugar, soft drinks, and alcoholic beverages. Also troublesome are greasy, spicy, and fatty foods. Alkaline foods are pretty much all vegetables and they are best consumed raw. Raw vegetables provide you with powerful enzymes that are necessary for good digestion.

Since I am on the subject of diet?it is important to recognize that obesity is one of the biggest contributors to GERD. If you have a weight problem then you can anticipate that acid reflux goes along with it. I know that it?s easier said than done, but losing weight is one of the most effective natural cures for heartburn that you will find.

Apples are also often recommended as a natural remedy for reflux. They are high in enzymes and can help soothe the fire in your belly.

One of the first natural remedies suggested to me actually came from my grandmother. She believed in the effectiveness of baking soda. Many people concur with her recommendation. Again, it?s not a cure-all but it may give you that immediate relief you seek.

Aloe vera juice is another natural remedy that some people feel good about. Once you get past the taste, which is worse than vinegar, it may give you some temporary relief.

Also on the list of potential remedies are milk, licorice root extract, acidophilus, and mustard. There are others?many others. But if you want the one alternative remedy that surpasses all GERD medications you will have the greatest success with changing your diet and losing weight.

Gannett reports lower 2nd-quarter earnings (AP)

NEW YORK ? Increases in digital and broadcast revenue were not enough to make up for the ongoing decline in Gannett Co.'s newspaper business, leading the publisher of USA Today to report a more than 22 percent drop in its second-quarter net income.

Still, the nation's largest newspaper publisher with more than 80 dailies said its results are showing improvement, and to show confidence in its business prospects, Gannett said it is resuming share buybacks and doubled its quarterly dividend.

Gannett said Monday that its net income fell to $151.5 million, or 62 cents per share, in the quarter ended June 26. That's down from $195.5 million, or 81 cents per share, at the same time last year.

Excluding one-time items, Gannett earned 58 cents per share. On this basis analysts polled by FactSet had expected adjusted earnings of 57 cents a share.

Revenue slipped 2 percent to $1.33 billion, close to the $1.34 billion that analysts were expecting.

"Overall, the results reflect the current state of economies here and in the U.K., strength in some sectors while (weakness) in others, a challenging environment for ad demand," said Chairman and CEO Craig Dubow in a conference call with investors.

Gannett said its company-wide digital revenue rose nearly 13 percent when compared with the year earlier, to $276.2 million. This consists of standalone digital businesses such as CareerBuilder.com, which Gannett said did well despite the anemic job market. It also includes digital revenue generated by other businesses, such as newspaper websites.

But its publishing division's advertising revenue, which counts both print and digital and makes up about half of the company's total, fell nearly 7 percent to $646.9 million. Print advertising revenue has been shrinking at most major newspaper publishers as advertisers turn to free or cheaper alternatives online.

Revenue at Gannett's broadcasting segment grew slightly to $184.4 million from $184 million. Last year's results were boosted by $11.7 million in political advertising that didn't happen this year.

In a sign of optimism, Gannett doubled its regular quarterly dividend to 8 cents from 4 cents. The company had cut its dividend sharply ? and for the first time ever ? from 40 cents in December 2008 amid the height of the economic meltdown.

In June, Gannett announced it is laying off 700 workers to cope with the ongoing advertising slump. This amounts to 2 percent of the company's work force, Gannett's biggest round of cuts in two years. Such ongoing cost cuts have helped the company to stay profitable for shareholders throughout the downturn.

Gracia Martore, Gannett's chief operating officer, said in the conference call that the company currently has "no intentions of looking at further reductions, but obviously that has to be driven by each individual business' prospects and revenue opportunities going forward." That said, she added that the company has been adding staff in key digital areas, as well as its broadcast division.

Shares of Gannett, which is based in McLean, Va., fell 43 cents to $13.05 in morning trading amid a broader market decline.
